Identity Theft occurs when someone takes your personal identifying information without your consent. "Personal identifying information" includes, but is not limited to, the following: name, address, telephone number, driver's license, driver's license number, commercial driver's license, commercial driver's license number, state identification card, state identification card number, social security card, social security number and birth certificate.

Contact the Federal Trade Commission Consumer Response Center to report identity theft, get a free credit report, register for do not call, etc.
To report identity theft, get your credit record (free for fraud victims) and to have it corrected, contact the three credit bureaus:
